FPDF / php modification needed (pq)

I need a coder experienced with FPDF to help me finish out a template.? Please see the attached zip file for file [login to view URL] I need to modify/add in the lines necessary to make the fpdf template above look like the file [login to view URL] (also in zip file).? I've added a few other similar (though? more complicated templates).? ? This is an extremely easy task compared to the other templates already done - they work fine.?
