Create a new page, using my template, within 6 hours

* I need to have this project completed in **_6 hours_**. I will be selecting a winner in less than **_1 hour_**. I have a template that is already built: <[login to view URL]> I need a contact page created from the template. It should be in HTML. Here are the things that I need done: - The page should be named [login to view URL] - Use a 2 column TABLE for the form - The text fields need to have the same gold border that they have in the PSD. - The scroll bar for the text area should be gold, as it is in the PSD - The up and down buttons of the scroll bar should be gold also - The Send and Cancel buttons need to be images, and look exactly like they do in the PSD Thanks, and HAPPY BIDDING!!! ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law.
