Form with phone validation to be built

Hi We are looking for a simple form to be built as a replacement for our website [login to view URL] that provides phone number validation and has steps to give the customer the chance to change their phone number as a replacement for our website [login to view URL] We are looking for our form to be made via ‘Bootstrap’ which can be downloaded from [login to view URL] . The form should mirror the form that is currently on [login to view URL] which also has some basic validation (name, email and phone number required, email address correct format and phone number in correct format). The validation for the form can be found at [login to view URL] [login to view URL] Once the form has been submitted, the form data should be sent off in an email and we would like the customer to go to a next step which asks the customer to verify their phone number. We have provided a wire diagram to demonstrate the steps involved for the customer to verify their phone number. The field in which the customer is asked to verify their phone number should be validated using a company called data8. Data8 check the phone number entered is a uk registered phone number.
