ON SITE REGISTER PHP and MySQL

I all ready have an On line Register, so I need to import this data to the OnSite system (and previous years data to the DB so we can prepopulate fileds to make registration faster) Front End STEP 1 We ask user on site if they have previously register on line of if this is the first time they register Because this is the 4 year of this event then we ask them if they attendee to last years event (this way we can just query their data and update it as a new attendee) (We use 3 ways to search them [login to view URL] EMAIL [login to view URL] Phone number [login to view URL] First Last name).. once we found it we can send it to print and the system will add it to a new list of (Time Stamp), as 2012 attendee. THE FIRST SCREEN IS NEW USER PRE REGISTERED USER IF IS A NEW USER THEN WE ASK THEM AGAIN IF THEY PREVIOUSLY ATTEND TO THIS EVENT this way we can get their info from the DB to marked as a 2012 attendee and print his label If this is their first year attendee the we ask for their info also marked as a 2012 attendee, print label and that is it. (LABEL PRINTING MUST BE FLEXIBLE TO CHANGE TO ME; BECAUSE I USE A COMPUTER AS THE MAIN DB RECEIVER AND LET THE LABEL PRINTER WORK AS NETWORK) THE BACKEND Must let me have 2 reports 1. Full 2012 Attende List (with all fiels exported as CVS or Excell) 2. Full DB export MUST BE SIMPLE TO INSTALL, NO INTERNET REQUIERED. One computer will work as server, SERVER AND 5 MORE COMPUTERS WILL BE LOADING AND PRINTING, MUST BE SUPER FAST AND ANTICRASH TEST. NOTE I will not take as a serious quote any bid that is just a copy paste response. I need full commitment from the programmer on this task so I can start working on future projects with it.
