Build website or app

I'm looking for a solution for an internal ordering system for my Stores department for consumables used by site operatives. No Ecommerce required, purely for internal ordering. Process I use just now. My site supervisors have a printed table of all the available stores we keep in stock. Items can be the likes of M10 bolts & nuts etc etc. They fill out writing the number of items they need in a Stores Call Up Sheet. They then send a photo of this with their phones to a Stores email. The Stores then have to print the photo which is very hard to see now and not very legible and then make up the order for dispatch. Process I want in place. I want an app or webpage so that they can make an electronic order of items they wish to order. This will also incorporate a filled out form which they have to fill out otherwise it won't process, such as contract name etc. The portal has to be accessible from a number of devices, app/android phones, iPads, laptops. It has to have a small image of the item and a quantity required box. It then should go into a basket, from where they fill out the information that is compulsory before sending to our Stores email or an alert of some sort for our Stores department to process. I want to be able to customize and administer this app/website myself to make changes and add items when needed. Find attached a copy of a Stores call up sheet we use & a workflow chart for clarity.
