Multi Select Dropdown in HTML Form to Multi Rows in MySQL DB using php and mysql

I can't afford to pay much more than $10 for this project. I am on a tight budget. Please bid low please. All I want to know is how to write php code that will take a multi select drop down within an HTML form (generated by php/mysql query) with multiple check boxes possibly selected with different values (with associated points) and send the ID#, values of the check boxes and associated points to multiple rows of all those selected within the multiple select drop down. In other words, if a user selects three people out of 10 from the drop down menu with various options then there should be three new records added to DB table with the ID, checkbox values, and points associated with check box values (this can be a total or separated per check box numeric value). I want to later build queries based on user ID and total points but this is not part of this project I just want to populate the table as described above for now. Please advise as to the best option for storing this data. I know ID needs to be a separate field, but the check box values and points...I don't know if they should be in separate columns or combined in single column. Thank you!
