PHP Page which will alter mysql tables

I need a simple web page (see attachment for layout) It will be used to compare data from 2 tables and alter one table after What I want is the following I have a table called blocked and one called cdr blocked has numbers only 77518745 45648754 54787445 54874548 etc CDR has call details 5478545 Date Time seconds (duration) 7845487 Date Time seconds etc, etc I will input a seconds value and hit generate ( with date range) eg seconds < 100 When I hit generate the page will use numbers from blocked table and grab corresponding ones on the cdr table (in range) , once located ; the script will find the the average of seconds of the numbers on the cdr table and display those which match, ( have an average of under 100 seconds) then print them out in the format shown in sketch I will then be able to purge those values FROM THE BLOCKED TABLE ONLY , no change to cdr table at all.
