PHP Form elements in jquery

I have a PHP-form that I need help with two seperate ajax-on-click features to do two actions before submit. I assume it takes 15 minutes in total if you know jquery by hand and 15-20 lines of jquery code. No MySQL or PHP-work required. If you know what you are doing, you can earn fast money. I'm a old-time PHP-developer withouth jquery/javascript knowledge, so I want to save 2-3 hours of my time by outsourcing this work, in addition to getting smaller/more effective code than I would be able to produce. So I'm looking for a jquery/javascript expert that can do this job in 24 hours max. Form already has jquery-script included. No external library or frameworks should be used (beside the jquery-libary of course). Two jobs: 1. Call an json-external-URL (jQuery ajax Method) when clicking on a button and respond with OK or FAIL inside the form. The url is called through curl. See [login to view URL] for form GUI. I already have this button working in javascript (onsubmit), but I need to do a jquery-ajax call instead) and you need to combine a country-dropdown, a text-field-number and radio-selected value in this call (the form elements is already there). 2. Add extra field. I need to be able to dynamically add more lines of dropdown and text-boxes just like image2.png. You can implement it using basic javascript like shown below (just using a dropdown and textbox instead of just one textbox): [login to view URL] You do not need to actually store any of the data or code any php, but I must be able to grab the selected dropdown-box value and textbox-value after POST submit of course. For instance as an array I can get to through $_POST. Simple and clean/short code is required.,
